Abberant Eyes (suggested). The otyugh odd position of sight organ allows the creature to spot approaching creatures with extreme accuracy, thus they gain advantage on any perception check based on sight.
Limited Telepathy. The otyugh can magically transmit simple messages and images to any creature within 120 ft. of it that can understand a language. This form of telepathy doesn’t allow the receiving creature to telepathically respond.
ACTIONS
- Multiattack. The otyugh makes three attacks: one with its bite and two with its tentacles.
- Bite. Melee Weapon Attack: +6 to hit, reach 5 ft., one target. Hit: (2d8 + 3) piercing damage. If the target is a creature, it must succeed on a DC 15 Constitution saving throw against disease or become poisoned until the disease is cured. Every 24 hours that elapse, the target must repeat the saving throw, reducing its hit point maximum by 5 (1d10) on a failure. The disease is cured on a success. The target dies if the disease reduces its hit point maximum to 0. This reduction to the target’s hit point maximum lasts until the disease is cured.
- Bite (suggested). Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit: (1d4 + 3) piercing damage. If the target is a creature, it must succeed on a DC 17 Constitution saving throw against disease or become poisoned until the disease is cured with a Lesser Restoration spell. Every 24 hours that elapse, the target must repeat the saving throw, reducing its hit point maximum by 5 (1d10) on a failure. The target dies if the disease reduces its hit point maximum to 0. This reduction to the target’s hit point maximum lasts until the disease is cured.
- Tentacle. Melee Weapon Attack: +6 to hit, reach 10 ft., one target. Hit: (1d8 + 3) bludgeoning damage plus (1d8) piercing damage. If the target is Medium or smaller, it is grappled (escape DC 13) and restrained until the grapple ends. The otyugh has two tentacles, each of which can grapple one target.
- Tentacle (suggested). Melee Weapon Attack: +8 to hit, reach 15 ft., one target. Hit: (1d8 + 3) bludgeoning damage plus (1d4) piercing damage. Limiting the power of a character and making the overall difficulty of the game harder, does not reduce the creativity, indeed it does quite the opposite.

DESCRIPTION
An otyugh is a bizarre and grotesque aberration that dwells in dark and filthy places, such as sewers, dungeons, and garbage dumps. It has a large, oval-shaped body covered with a tough, rocky hide that can blend in with its surroundings. It stands on three thick, stumpy legs that allow it to move with surprising speed. It has a huge mouth full of sharp teeth that can inflict deadly diseases with its bite. It also has three long tentacles that end in spiky pads. Two of these tentacles are used to grab and smash prey, while the third one has a pair of eyes and a nose on top of a stalk. This sensory tentacle allows the otyugh to see and smell its environment, as well as communicate telepathically with other creatures.
COMBAT
An otyugh is not very aggressive, but it will defend its territory and food source from intruders. It prefers to ambush its prey by hiding under piles of trash and debris, exposing only its sensory tentacle. When a potential meal comes close enough, it lashes out with its other tentacles, trying to grapple and crush its victim. It then drags the prey into its mouth, where it delivers a powerful bite that can cause infections and diseases. An otyugh can also use its limited telepathy to lure or deceive unsuspecting creatures, or to bargain for food or friendship.
HABITAT / SOCIETY
Otyughs are usually solitary creatures, but they can form loose alliances with other otyughs or creatures that share their habitat. They are somewhat intelligent and can understand common language, but they have a very different worldview and morality than most humanoids. They see themselves as the cleaners and recyclers of the world, consuming waste and turning it into fertilizer. They have no concept of beauty or hygiene, and they enjoy living in filth and decay. They are also very curious and greedy, always looking for new things to eat or collect. Otyughs mate once a year for a month, producing one offspring that stays with its parent for a few years before leaving to find its own lair.
ECOLOGY
Otyughs are omnivorous scavengers that can eat almost anything organic, including carrion, offal, feces, fungi, plants, and even metal or wood. They have a very efficient digestive system that breaks down most substances into nutrients and minerals. They excrete a foul-smelling sludge that enriches the soil and promotes plant growth. Otyughs are also immune to most diseases and poisons, but they can carry and transmit them to other creatures through their bites or contact. Otyughs have few natural predators, as most animals avoid them due to their smell and appearance. However, they are sometimes hunted by adventurers or explorers who seek their valuable hide or organs.
