Cloud of Vermin. Any creature, other than a devil, that starts its turn within 20 feet of the nupperibo must make a DC 11 Constitution saving throw. A creature within the areas of two or more nupperibos makes the saving throw with disadvantage. On a failure, the creature takes 2 (1d4) piercing damage.

Hunger-Driven. In the Nine Hells, the nupperibos can flawlessly track any creature that has taken damage from any nupperibo’s Cloud of Vermin within the previous 24 hours.

Regeneration (suggested). A nupperibo regenerates 1 hit point per round until reduced to zero hit points.

5th Edition Advanced Mode
Limiting the power of a character and making the overall difficulty of the game harder, does not reduce the creativity, indeed it does quite the opposite.
The Game Master has the option to use any and all of the instances proposed in this guide, or just some of them according to their preference.

It is the lack of something that move and motivate characters, not the abundance of it

Nupperibo

A nupperibo is a type of fiend that inhabits the Nine Hells, the plane of lawful evil. It is one of the lowest forms of devils, barely above the mindless lemures. A nupperibo is a bloated, blind creature that shuffles aimlessly across the infernal landscape, seeking anything to devour. It is surrounded by a cloud of vermin that torments any non-devil that comes near it. A nupperibo has no memory, no personality, and no purpose other than to suffer and feed.

Description

A nupperibo resembles a vaguely humanoid blob of flesh, about as tall as a human. It has dark grayish-black skin that is covered with boils, scars, and wounds. It has no eyes, nose, ears, or hair, only a gaping mouth full of jagged teeth and a pair of clawed hands. It wields a crude spear that it uses to stab at anything that moves. A nupperibo emits a constant moan of pain and hunger that can be heard from far away.

A nupperibo is not alone in its misery. It is constantly accompanied by a swarm of rats, flies, maggots, and other vermin that crawl over its body and bite at its flesh. The vermin also form a cloud around the nupperibo, obscuring its vision and spreading disease to anyone who approaches it. The nupperibo does not seem to mind the vermin, and sometimes even eats them when it can catch them.

A nupperibo is a corrupted soul that was sent to the Nine Hells after death. It was once a mortal who committed evil acts in life out of carelessness or sloth, rather than malice or ambition. Such souls are deemed worthless by the devils, who transform them into nupperibos as a punishment and a mockery.

A nupperibo is also known as a baatezu petitioner, slightly higher in station than the lemures. It is used as fighting, feeding, and tormenting stock by all baatezu from the pit fiends on down. A nupperibo has no loyalty or allegiance to any devil lord or faction, and will attack any non-devil on sight. A nupperibo has no concept of hierarchy or rank, and will not obey any orders or commands.

Combat

A nupperibo is not a skilled or intelligent combatant. It relies on its brute strength and its vermin cloud to attack its enemies. A nupperibo can sense any creature that has taken damage from its vermin cloud within the previous 24 hours, and will pursue it relentlessly until it is either killed or fed. A nupperibo can also bite with its powerful jaws, but prefers to use its spear for longer reach.

A nupperibo is resistant to acid, cold, and nonmagical weapons that are not silvered. It is immune to fire, poison, and several mental conditions. It has blindsight up to 10 feet, but is blind beyond that radius. A nupperibo has no sense of self-preservation or fear, and will fight until it is destroyed.

When a nupperibo is destroyed, it reforms into another nupperibo. However, there is a 1% chance that it becomes a lemure. Although this means a reduction in station (however slight), it also means a chance to become a spinagon in the future.

Habitat / Society

A nupperibo can be found in any layer of the Nine Hells, but is most common in the first layer, Avernus, where it wanders among the ruins and battlefields of the Blood War. A nupperibo rarely interacts with other nupperibos, except to fight over food or territory. Sometimes, a group of nupperibos will form a horde that rampages across the landscape, devouring anything in their path.

There is, however, a unique and curious relationship between the lemures and the nupperibos. The nupperibos are slightly higher in station than the lemures, but they can never become higher forms of baatezu without first being demoted to lemure status. Doubtless some greater power in Baator has set the advancement path that way for its own fiendish reasons.

Nupperibos are commonly used as trading stock to the yugoloths in return for their mercenary services. Like the baatezu, the yugoloths treat the nupperibos horribly and ultimately consume them.

Ecology

A nupperibo feeds on anything organic that it can find, such as corpses, plants, animals, or other fiends. It has no preference or taste for any type of food, and will eat anything that fills its stomach. A nupperibo does not need to sleep or rest, but will sometimes lie down when it is not hungry or threatened.

Whether left in Baator or traded to yugoloths, nupperibos lead a completely wretched existence. Lower planar creatures consider them insignificant.
