Aggressive. As a bonus action, the ore can move up to its speed toward a hostile creature that it can see.

Corrupted Carrier. When the orc is reduced to 0 hit points, it explodes, and any creature within 10 feet of it must make a DC 13 Constitution saving throw. On a failed save, the creature takes 14 (4d6) poison damage and becomes poisoned. On a success, the creature takes half as much damage and isn’t poisoned. A creature poisoned by this effect can repeat the save at the end of each of its turn, ending the effect on itself on a success. While poisoned by this effect, a creature can’t regain hit points.

Nurtured One of Yurtrus. The orc has advantage on saving throws against poison and disease.

5th Edition Advanced Mode
Limiting the power of a character and making the overall difficulty of the game harder, does not reduce the creativity, indeed it does quite the opposite.
The Game Master has the option to use any and all of the instances proposed in this guide, or just some of them according to their preference.

It is the lack of something that move and motivate characters, not the abundance of it

DESCRIPTION

An Orc Nurtured of Yurtrus is a humanoid creature that has been infected and transformed by a plague that is spread by the priests of Yurtrus, the orc god of death and disease. Yurtrus is one of the many orc deities that influence the behavior and culture of different orc tribes. The priests of Yurtrus, also known as the hands of Yurtrus, isolate the sick orcs from the rest of the tribe and nurture them with their dark magic, turning them into living weapons of war. The Orc Nurtured of Yurtrus have grayish skin that is covered with boils, sores, and lesions. Their hair is coarse and matted, and their eyes are dull and bloodshot. They have a low forehead and a pig-like face, with lower canines that resemble boar tusks. They also have ears that are pointed at the end. They wear ragged and filthy clothes, and carry no weapons or armor. They are driven by a mindless rage and a desire to spread their disease to others.

COMBAT

An Orc Nurtured of Yurtrus is a dangerous foe that can inflict both physical and poison damage to its enemies. They are aggressive and fearless, charging into battle with their claws and teeth. They have no regard for their own safety, and will often sacrifice themselves to cause more harm to their opponents. When an Orc Nurtured of Yurtrus is reduced to zero hit points, it explodes in a burst of pus and blood, releasing a cloud of toxic spores that can poison any creature within 10 feet of it. A creature that is poisoned by this effect can’t regain hit points and must make a saving throw at the end of each of its turn to end the effect. The Orc Nurtured of Yurtrus is immune to poison and disease, and has advantage on saving throws against them. They are also resistant to necrotic damage, as their bodies are already corrupted by the plague.

HABITAT / SOCIETY

An Orc Nurtured of Yurtrus is usually found in the lairs of orc tribes that worship Yurtrus, the god of death and disease. They are kept in dark and damp chambers, where they are tended by the priests of Yurtrus. The priests cultivate the sickness of these nurtured ones, using them as instruments of defense and weapons of war. The Orc Nurtured of Yurtrus are not respected or valued by the rest of the tribe, and are seen as disposable pawns. They have no social status or role, and are treated as objects rather than living beings. They have no sense of self or identity, and are driven by a primal instinct to spread their disease to others. They are sometimes used as scouts or shock troops, sent ahead of the main orc force to soften up the enemy ranks. They are also used as suicide bombers, sent to infiltrate and destroy enemy fortifications or settlements.

ECOLOGY

An Orc Nurtured of Yurtrus is a creature that has been altered by a magical plague that is spread by the priests of Yurtrus. The plague is a form of divine curse that is bestowed by Yurtrus, the orc god of death and disease. The plague affects only orcs, and is transmitted through contact with infected blood or bodily fluids. The plague causes the infected orc to develop symptoms such as fever, chills, nausea, vomiting, diarrhea, and weakness. The orc’s skin also becomes covered with boils, sores, and lesions, and its hair falls out. The orc’s mind also deteriorates, becoming more aggressive and irrational. The orc loses its sense of self and identity, and becomes a mindless servant of Yurtrus. The plague also grants the orc some benefits, such as immunity to poison and disease, resistance to necrotic damage, and the ability to explode upon death. The plague is incurable by mundane means, and can only be removed by a powerful magic such as a wish spell. The Orc Nurtured of Yurtrus has no natural predators or allies, and is hostile to any creature that is not infected by the plague. The Orc Nurtured of Yurtrus does not need to eat, drink, or sleep, and does not reproduce. The Orc Nurtured of Yurtrus is a creature that exists only to spread death and disease in the name of Yurtrus.
