Deadly Stalker. If the avatar should attack while remaining undetected (either hidden or dis-guised), the victim suffers disadvantage on his surprise roll. One member of a surprised group will be singled out and must roll a DC 23 dexterity saving throw or be immediately strangled to death when the avatar slips a knotted cord around his throat.
